Reading a thyroid panel without panicking

TSH slightly out of range is common, frequently temporary, and rarely a reason to start medication that week.

Thyroid results cause more unnecessary worry than almost any other panel. TSH is sensitive — it moves with illness, stress, pregnancy, time of day and the biotin in your hair supplement.

One reading is not a diagnosis

A TSH of 5.2 with normal free T4 and no antibodies usually warrants a repeat in six to eight weeks, not a prescription. Roughly half of those readings come back normal on their own.

When it does matter

Persistent elevation with low free T4, or raised TPO antibodies, is a different conversation — that pattern is worth acting on, and worth an endocrinologist’s eye.

Either way, we run the full panel, because TSH alone cannot tell those two stories apart.
